Celebrating Success Awards 2022: Nominate Below! *Deadline Extended To 20 September

Basic Rules

For the 2022 Celebrating Success Awards, any person supported by Praxis Care can be nominated based on their achievements in the last year. A person may be nominated in and win more than one category.

Nominators can include anyone who knows the person they are nominating: staff, family member, friend, professional support, etc.

The judging panels will be reviewing applications based on an individual’s success relative to their personal goals and aspirations. Please include as much detail as you can within 200 words in the nomination to demonstrate how the person has progressed in the past year.

Shortlisting

All nominees will receive a certificate.

Nominees will be divided into 3 groups regionally for shortlisting: (1) Great Britain and Isle of Man, (2) Northern Ireland and (3) Republic of Ireland.

Each region will shortlist up to 3 people per category. Shortlisted nominees will be informed in October 2022 and will be invited to an awards ceremony within their region to be held in November/December 2022 where the winners will be revealed.

All shortlisted nominees will receive a cash prize with the ultimate winners receiving £200/€200!

Categories

There are 5 categories.

The Co-Production Award

• This award celebrates great achievements that were co-produced.
• Achievements by sharing ideas, views and making a meaningful difference with the help of others.
• The winner’s effort and involvement has helped them personally and/or helped others.

The Health and Wellbeing Award

• Achieved, maintained and promoted a healthy lifestyle.
• Made a significant change to their lifestyle which improved their personal health and taken up a hobby to support positive health and well-being or given something up.

The Community Involvement/Active Citizen Award

• Makes a significant contribution to their local community.
• Actively participates in their community thus impacting on positive outcomes for themselves or others.

The Person of the Year Award

• This award recognises the exceptional achievements of an individual
• Has achieved excellent outcomes that have enhanced their quality of life or the quality of life of others.
• Has achieved significant measurable progress in the last 12 months with their individual goals.

The Praxis Talent Award

For example:

• Musician of the year
• Artist of the year
• Chef of the year
• Gardener of the year
• Fundraiser of the year
• Our place of the year
• Recycling champion of the year

This award recognises the many exceptional talents and skills at Praxis Care. We welcome nominations with supporting evidence that showcase the talent of the people we support. (With their permission, we would also like to showcase this work also.)
