Foyle Floating Support is a floating support service for adults.
We support adults over 65 with complex needs, such as addictions, mental ill health and/or physical health issues, as well as individuals under age 65 with a dementia or brain injury diagnosis.
We provide short-term Floating Support services delivered to individuals in their homes to maximise independence with housing-related support. Floating Support is a short-term funded service, meaning people may receive support from a few weeks up to a maximum of two years.
We provide practical and emotional support to help develop daily living skills, improve health and well-being, increase personal resilience and maintain independence.
The type of support can include:
- Housing – Finding a home – Setting up and maintaining your home
- Support to make sure home is safe and secure
- Support with emotional wellbeing & physical health – maintaining a healthy lifestyle
- Support to manage money/benefits advice
- Developing independent living skills – meal planning
- Build positive relationships/ support networks and the local community
- Support to gain access to other social, recreational activities
This service is funded by Supporting People and the Western Health and Social Care Trust.
