Greater Manchester Complex Needs Community Supported Living Service
The Greater Manchester Complex Care Supported Living Service (GMCC) is a supported accommodation service for adults located in within Greater Manchester.
We support adults with learning disabilities and/or mental ill health.
Referrals to services involve detailed pre-admission assessment, relationship and knowledge building utilising clinical input, working closely with each individual, their multidisciplinary teams and stakeholders and engaging with families through a managed transition process.
- We provide:
Supported Living accommodation that is bespoke to each individual’s needs. The majority will be single occupancy staffed 24/7. - We have significant experience of co-designing and implementing successful transition plans for people assessed as having very complex needs moving into community-based settings or transitioning into adult services from children’s services.
• Our transitions are safe, managed, coordinated, bespoke and effective.
• In-house property and development teams work closely to source and adapt appropriate accommodation in a person’s preferred location
• Properties will have easy access to transport and local amenities within the Greater Manchester Area
Referrals can be made by professionals such as statutory keyworkers, GPs and social workers.
